Wedge-shaped defects
The V-shaped notches near the gum line: what causes them and how they are treated.

A wedge-shaped defect is a notch at the neck of the tooth, not caused by decay. Aggressive horizontal brushing, bite overload and acid erosion all contribute. The area becomes sensitive and, over time, weak.
Treatment
- Small defects: fluoride and a change of brushing technique.
- Larger defects: a composite restoration that seals the dentine and restores the contour.
- Bite analysis when grinding or overload is the cause.
